This small ornament chart was published in February, 1994, under the Needle Adventures name. It appears to be one of the Christmas Heirloom ornament series. (Note the green lettering on white paper.)

7x7 gray Congress cloth
Needle Necessities overdyed perle #853 (substitute ThreadworX 81089 or 81090 size 8 perle cotton)
Supertwist #15 (thin metallic from Madeira similar to Accentuate)
Balger 1/16 inch metallic ribbon red #003 (Balger is the old name for Kreinik metallics)
